This is the page where we most want to talk you out of the expensive purchase. If reflux at night is the problem, a wedge pillow costs a small fraction of a base and will tell you in a week whether elevation is the answer. The base answers exactly the same question, an order of magnitude more expensively, and later.
So the picks here start with the cheap ones on purpose, and the bases are further down.

What elevation is doing
Lying flat puts the stomach and the esophagus at the same level. Raising the upper body uses gravity to keep stomach contents where they belong. That is the whole mechanism, and it is why the shape of the elevation matters more than the amount.
Raising the head is not the same as raising the torso. A stack of pillows folds the neck forward and leaves the chest more or less flat, which achieves very little and can make things less comfortable. A wedge, or a bed that raises from the hips, lifts the whole upper body as a unit. Angles, and how to know what you are actually at.
Where the 30-degree number comes from
Almost every article on this subject repeats 30 degrees without saying why. The number is a Medicare coverage criterion. Coverage determination L33820 lists, among the criteria for a hospital bed, a beneficiary who requires the head of the bed elevated more than 30 degrees most of the time due to congestive heart failure, chronic pulmonary disease, or problems with aspiration.
That is a threshold for equipment funding, not a prescription for everyone with heartburn. It is useful to know because it is where the internet's favorite number came from, and because it is directly relevant if you are pursuing coverage. Our Medicare page goes through the criteria in full.

The third option nobody mentions
Raising the head end of the entire bed on risers is an old approach and it has a genuine advantage over a wedge: your body stays straight, so there is no fold at the hips and nothing to slide down. It uses two risers rather than four, so one cheap set leaves you spares.
Two cautions. Check the bed is stable on a slope before you sleep on it — rock it hard, with the mattress on. And be aware that you will tend to migrate down the bed over the night, which some people mind a great deal and others do not notice.
If the wedge worked
Then you know elevation helps, and the question becomes whether you want to live with a wedge or with a bed that does it invisibly. The honest comparison is here, including the cases where the wedge simply wins on the merits.
Questions people actually ask
How high should I raise my bed for acid reflux?
The commonly quoted 30-degree figure comes from Medicare's hospital bed coverage criteria, not from a universal clinical recommendation. What matters mechanically is that the whole upper body is raised rather than the head and neck being folded forward on pillows.
Is a wedge pillow or an adjustable bed better for reflux?
A wedge is the right first purchase: it costs a fraction and tells you within a week whether elevation helps. A base is better for living with it long term, because it holds the angle without sliding and lets you change it. The full comparison.
Can I just use extra pillows?
It is worth trying, but stacked pillows tend to bend the neck rather than raise the torso, and they slide out during the night. That is the specific problem a wedge is shaped to solve.
Do bed risers under the head of the bed help reflux?
Raising just the head end of the whole bed is a long-standing approach and it keeps the body straight rather than folded. Riser sets are cheap and this uses only two of the four. Do check the bed is stable on a slope before sleeping on it.
Are wedge pillows FSA eligible?
Some are, and it is stated on the listing when they are — one of the two here says so explicitly. Eligibility is product-specific, so check before assuming. More on HSA and FSA.
