HSA and FSA eligibility

Usually not for a bed, sometimes for what goes on it. What actually decides eligibility, and what a letter of medical necessity does.

Short version: a bed frame, usually no. Specific products bought for a specific diagnosed problem, sometimes. And the person who decides is your plan administrator, not a website and not the seller.

What makes something qualify

Health savings accounts and flexible spending accounts pay for qualified medical expenses. The general test is whether the item is for the diagnosis, cure, mitigation, treatment or prevention of disease — rather than for general health or comfort. IRS Publication 502 is the underlying reference for what counts.

An adjustable bed base fails that test the same way it fails Medicare's: it is a comfort product anyone might buy. The fact that it happens to help with a condition does not by itself make it a medical expense.

What a letter of medical necessity does

For dual-purpose items — things with both a medical and an ordinary use — many administrators will consider a letter of medical necessity from a treating clinician. It typically names the diagnosis, the item, and why the item is needed to treat or alleviate the condition.

Two things worth understanding. It is a request, not an entitlement — the administrator still decides. And it is worth obtaining before you buy, because a retrospective letter for a purchase already made is a harder conversation.

The practical approach

  1. Ask your administrator first, describing the specific item rather than the category. Get the answer in writing.
  2. Check the listing. Some products state FSA eligibility explicitly — one of the wedge pillows we track does — and that is a meaningful signal about how the item is generally treated.
  3. Ask the clinician for the letter before buying, if the item is dual-purpose and the amount is material.
  4. Keep everything. The receipt, the letter, the administrator's reply. Substantiation requests come months later.

And the cheaper point that keeps coming up

Eligibility is worth chasing on a base. It is not worth chasing on a set of risers — and risers are frequently the thing that actually solves the problem. If you have not done the height arithmetic yet, do that before you spend an afternoon on paperwork for the wrong purchase.

Are adjustable bed frames HSA eligible?

Generally not on their own. An HSA or FSA covers qualified medical expenses, and ordinary furniture that anyone might buy does not qualify. A letter of medical necessity from a treating clinician can change the analysis for some items, and your plan administrator makes the final call.
