Before the products, the test. It costs nothing, it takes one night, and it answers the only question that matters here.
That position — knees slightly above the heart, head slightly raised — is what gets sold as zero gravity. It is a saved position on a remote, not a distinct capability. Every base with head and foot lift can reach it whether or not it has a button labeled for it.

What the position actually does
Lying flat on your back leaves a gap under the lumbar curve, and the muscles either side hold that curve all night. Raising the knees lets the pelvis rotate slightly and the lower back settle toward the mattress, which for many people reduces that constant low-level load.
That is a mechanical description, not a clinical claim. Back pain has many causes and some of them get worse with the wrong position. If your pain is new, severe, or comes with numbness or weakness, that is a doctor rather than a bed.

The order to spend in
- Nothing. Pillows, for a week. If the position helps, continue; if it does not, stop here.
- A topper. If the problem is the surface rather than the angle — an old mattress that has softened in the middle — a topper is the cheap test of that too.
- A base. If the angle helps and you want it held properly every night. The full comparison.
- A mattress. Only if yours is genuinely worn, or will not flex on a base. How to tell.
One thing that is easy to miss
If your back hurts most when you are getting out of bed rather than while you are in it, the angle is not the variable — the height is. Rising from a bed that is too low means standing up out of a squat, which loads the lower back heavily. Measure your target height before assuming you need a base.
And if only one of you needs the position, a split king means one side changes and the other does not. Two mattresses and a gap filler to budget for, but nobody has to compromise.
Questions people actually ask
Do adjustable beds help back pain?
Some people find that raising the knees slightly reduces lower back discomfort by letting the lumbar spine flatten. Whether it helps you is testable for free tonight with a pillow under your knees. If that helps, a base makes it repeatable; if it does not, a base will not change that.
What is the zero-gravity position?
A saved position with the knees slightly above the heart and the head slightly raised. It is a preset, not a function — any base with head and foot lift can reach it by hand.
Is a firm or soft mattress better for back pain?
There is no consensus figure and firmness labels are not standardized between manufacturers. What we can say without inventing anything is that a sagging mattress makes getting in and out harder, and that a topper is the cheapest way to change the surface without replacing the mattress.
Should I buy an adjustable bed for sciatica?
That is a question for whoever is treating it. What we can tell you is what a base does mechanically and what it costs. Anything beyond that would be us pretending to a clinical opinion we do not have.
