The orthopaedic surgeons' own patient page on low back pain does not name a mattress firmness. Here is what we score instead, and why.

Almost every adult will experience low back pain at some point, as the American Academy of Orthopaedic Surgeons puts it on its own patient page. What that page does not do - anywhere on it - is name a mattress, a firmness or a sleeping surface. That absence is the honest starting point for this comparison, and it is the reason we score these three products on things that are published rather than on a word that is not defined.

Why we will not tell you a firmness
Firmness labels are not standardized. There is no scale behind medium firm, no test a manufacturer has to pass to use it, and no way to compare one brand's medium with another's. Two of the three mattresses here are labelled medium firm and medium by their own sellers, and those two words are doing no work at all. A page that tells you back pain needs a medium-firm mattress has taken an undefined word and attached a medical claim to it, and we are not going to do that.
What can be said usefully is narrower and more boring. A mattress that has lost support where you sleep holds your spine in the shape of the hollow all night, and that is a mechanical fact about the mattress rather than a claim about your back. A mattress you cannot get out of easily makes the stiffest moment of the day worse. And a mattress that will not flex is the wrong buy if you own or intend to buy an adjustable base. Those three things are checkable, so those three things are what we score.
What we score, in order
- Published height. Because thickness is more than half the distance from the floor to where you sit, and because the height of the finished bed is what decides whether the first movement of the day is a step or a struggle. All three mattresses here publish 12 inches.
- Whether the listing states base compatibility in writing. A mattress that will not bend is useless on an adjustable base and there is no way to tell from a photograph. One of the three states it outright; the others leave you to infer it from the construction.
- What the construction actually is. All-foam, layered foam, hybrid - published, not guessed. This is where the support question lives, and it is also where most listings go quietest.
- Certification, and what it does and does not cover. CertiPUR-US covers what the foam is made of and emits. It says nothing about durability, comfort or how long support lasts. It is worth having and it is not a quality rating.
- Published weight limit and warranty, where they exist. Only one of the three publishes either. Support failing early is the thing that causes the back problem this page is about, and a warranty term is the nearest thing to a manufacturer's own estimate of when that happens.

The Zinus is the one whose listing publishes the most in writing - stated compatibility with platform beds, box springs, slatted frames and adjustable bases, a named carbon-rayon flame barrier in place of fiberglass, an OEKO-TEX cover, a 500 lb weight limit in queen and a 10-year limited warranty. If you value a manufacturer committing things to print, that is the one, and on this page that counts for a lot.
Why a wedge pillow is on a mattress page
Because for a lot of people with lower back pain, the thing that helps is a change of shape rather than a change of surface, and the cheapest way to find out is a wedge under the knees for a week. It publishes a 7.5-inch rise and FSA eligibility on its own listing, and it costs a small fraction of a mattress. If it helps, you have learned that position is your lever, which points at an adjustable base rather than at a new mattress. If it does nothing, you have ruled something out cheaply and you can spend the money on the surface with more confidence.
That order - cheap test first, expensive purchase second - is the one we would follow ourselves, and it is the opposite of how this category is usually sold.
The one we would skip for back pain
The topper, and only in this context. It is a good product and it appears on our topper comparison as the one we would buy, because for a firm mattress that simply feels too hard it is the right answer for a tenth of the money. For lower back pain it is the most common wrong purchase in the whole category, for a specific mechanical reason: if your mattress sags where you sleep, a topper follows the dip. You are adding a softer layer over the hollow that is holding your spine in a bad shape, which makes the surface feel kinder for a fortnight and changes nothing about what is underneath.
There is a second reason to be careful with it here. A 3-inch topper raises your sleep surface by 3 inches, so a bed you had set correctly becomes 3 inches taller, and getting out of it in the morning gets harder - at exactly the moment a painful back least needs that. Nobody thinks of a topper as a height decision, because it arrives in a comfort-shaped box. The test for whether your mattress is the problem takes two minutes and is the thing to do before buying either.

How any of these feels. We have not slept on them, we do not have a test lab, and units we claim to have tested: zero. Everything above comes from the published listings, read on the dates stamped on each specification table, plus the AAOS patient page cited in the sources. We could not verify how long support lasts on any of the three, because only one publishes a warranty term and none publishes a durability figure. And we could not tell you which firmness suits your back, because nobody can - that is a conversation with a doctor or a physiotherapist who can examine you, and this page is not a substitute for one. Questions people actually ask
What is the best mattress for lower back pain?
There is no published firmness that is correct for back pain, and any page that gives you one is inventing it - the AAOS patient page on low back pain does not name a mattress at all. What is defensible is this: a surface that supports you evenly rather than one you sink into unevenly, at a height you can get out of. Is a firm or soft mattress better for back pain?
Nobody can tell you, because firmness labels are not standardized. One manufacturer's medium firm is another's medium and there is no scale behind either word. What is measurable is whether the mattress still supports you - a mattress with a hollow where you sleep holds your spine in the shape of that hollow for eight hours, whatever the label on it says.
Will a mattress topper help my back pain?
Only if the mattress underneath is structurally sound and merely feels too hard. If it sags where you sleep, a topper follows the dip and adds softness over the same hollow - it is the most common wrong purchase in this category. When to replace instead of adding.
Does an adjustable bed help back pain more than a mattress?
It changes a different variable. A base changes the shape you lie in; a mattress changes what supports that shape. Many people find a slight knee raise with a slight head raise eases the lower back, and you can test that tonight with pillows before spending anything. What a base does for back pain.

More than most pages admit. The stiffest moment of the day is usually getting out of bed, and how high the sleep surface sits decides whether that is a step or a heave.
