Medicaid and VA routes

Most articles on this subject stop at Medicare. Two other routes exist, they work differently, and each is worth ten minutes before concluding that nobody will help.

A caution up front: we are deliberately not going to tell you what your state covers or what you personally qualify for. Medicaid rules vary state by state and VA eligibility depends on enrollment and clinical referral. What we can do is tell you which door to knock on and what to ask.

Medicaid

Medicaid is jointly funded by the federal government and the states and administered by each state within federal rules. Durable medical equipment is a benefit category, but what is covered, under what criteria and with what prior-authorization process differs between states.

The practical consequence is that a categorical answer about adjustable beds and Medicaid does not exist — anyone who gives you one nationally is guessing.

• Start with your state Medicaid agency's durable medical equipment policy, or ask a caseworker directly.
• Ask specifically about hospital beds and accessories, using the equipment vocabulary rather than the word adjustable — the codes are here.
• If you have both Medicare and Medicaid, ask how they coordinate. Medicaid may cover cost sharing Medicare leaves you with.
• Ask about prior authorization early, because it is usually the step that takes the time.

The VA

Veterans enrolled in VA health care can receive durable medical equipment through the Prosthetic and Sensory Aids Service. The general position is that eligibility runs through enrollment in VA health care plus a medical need for the item, established by a VA provider's referral.

• The route starts with a VA provider, not with a catalog or an outside supplier.
• A VA social worker is usually the most efficient person to ask, and they can see the whole picture including home care.
• Veterans service organization representatives help with this at no charge and know the system well.
• Equipment provision is separate from any disability compensation question — do not assume the answer from one applies to the other.

What to ask, in any of these systems

1. Is the equipment I need in a covered category at all? If it has no billing code, no amount of documentation will help.
2. What clinical criteria apply, and where are they written down?
3. What documentation does the clinician need to provide?
4. Is prior authorization required, and how long does it take?
5. Which suppliers are approved, and do they accept assignment?
6. If it is denied, what does the appeal process look like?

And the practical fallback

If none of these routes work, the situation is the same as the one at the end of our Medicare page: work out what problem you are actually solving, and spend the smallest amount that solves it. That is frequently a height correction rather than a bed at all.
