The massage function on an adjustable bed is a set of small vibration motors mounted in the head and foot sections of the deck. Press the button and the frame buzzes, and the mattress carries some of that to you. It is vibration, not kneading, and no base in this comparison publishes anything that suggests otherwise. That is the entire feature, and knowing it is the entire feature is most of what you need before you shop.

What we are not going to tell you
We are not going to tell you that it treats anything. Even for massage therapy as practiced by a person with hands, the National Center for Complementary and Integrative Health describes the research as thin — and a vibration motor bolted under a steel deck is not massage therapy in that sense at all. No listing we read makes a clinical claim for it either, which is to their credit. So the honest framing is that this is a comfort feature some people enjoy, it costs the manufacturer very little, and it appears on the box because it photographs well.
There was evidence that massage was helpful for chronic low-back pain but the strength of the evidence was low.
That framing is worth holding onto, because massage is the most heavily advertised feature in this category and the one least likely to decide whether the bed works for you. The specifications that decide that are leg height, published capacity and how usable the controls are with stiff hands. Massage is a tiebreaker.

What to check before you buy for the massage
If you have decided you want it, these are the questions worth putting to the seller in writing before you order, because no listing in this comparison answers them:
- Does it run in both the head and the foot, or only one? Three listings above specify head and foot. The rest do not say, and a foot-only unit is a different product from what most people picture.
- Does it time out, and can that be changed? Most units shut off automatically after a set period. Nobody publishes the period.
- How loud is it, and can it be heard on the other side of a split king? Not one of the seven publishes a figure for this. If a partner sleeps beside you, ask.
- Can the massage run while the bed is flat? On some units it is tied to the position controls. If you want it while lying flat, confirm it.
- Is the massage motor covered for as long as the lift motor? The DOMIA and both Tami bases publish five-year warranties; most of the rest publish no warranty term at all, and warranties routinely cover different parts for different lengths. What warranties actually cover.

- Motor noise given as a number, under 45 dB, rather than as the word quiet. Only two other bases in this set — both Tami Tami split kings — do that

If massage is why you are here, though, it is the wrong buy: its massage specification is the bare word Yes, it publishes nothing whatsoever about leg height or frame height, it has no published presets, and at 137 pounds of frame it is a two-person carry. You would be paying for a feature described in one word, on a base that is silent about the specification this site cares about most.
Who actually gets something out of it
Two groups, in our reading of what the feature is. People who find low-level vibration relaxing as a way of getting to sleep, which is a taste rather than an outcome and is a perfectly good reason to want it. And people whose legs will not settle at night and who find a distraction helps — though that is highly individual, and some people with restless legs find vibration makes it worse rather than better. There is no way to know which you are without trying it.
If what you actually want is something done about swollen legs, the useful control on these beds is the foot lift, not the massage motor, and what decides the result is how long the elevation is held. Elevation and circulation covers that properly. If what you want is help getting out of bed in the morning, neither button matters and the height of the bed decides everything.

What does the massage function on an adjustable bed actually do?
It runs small vibration motors built into the head and foot sections of the deck. It is vibration through the mattress, not kneading, and nothing in this price bracket does more than that. Whether you like it is a matter of taste; it is not a therapy, and no listing we read claims it is.
Which adjustable beds have massage?

Is massage worth paying extra for on an adjustable bed?
We would not choose a base for it. It is the feature owners describe using enthusiastically for a fortnight and rarely after that, and it sits on bases that are otherwise ordinary. If two bases are equal on leg height, controls and published capacity, take the one with massage. Do not take a worse base to get it.
Is bed massage good for restless legs or circulation?
No listing we read publishes evidence that it treats either, and we are not going to imply otherwise. Some people with restless legs find vibration distracting in a useful way and some find it maddening. What a bed can and cannot do for restless legs, and what foot elevation does for swelling, are both more substantial than the massage motors.
Do the massage motors make noise?
