Leg elevation for swelling and circulation

Above the heart is the phrase everyone uses. Here is what that means in a bed, and why pillows stop working after an hour.

Swollen ankles and lower legs at the end of the day are common, and elevation is the standard piece of self-management advice. The advice is usually given as three words — above the heart — and almost never explained in terms of an actual bed.

What above the heart means lying down

Lying flat, your heart is roughly at chest level and your feet are at the same height. To get the feet above the heart you have to raise them by more than most people picture: the calves and feet need to sit noticeably higher than the chest, with the knees slightly bent so the hamstrings are not stretched all night.

Two pillows under the calves produces perhaps four inches, in a shape your legs will slide off. That is the gap between the advice and the reality of a bed.

Why pillows fail overnight

  • They compress. A pillow under load loses much of its height within twenty minutes.
  • They migrate. Legs move at night, and a loose pillow ends up beside them rather than under them.
  • They create a pressure line. A stack under the calves concentrates pressure at one point rather than supporting the whole leg, which for someone with fragile skin is its own problem.
  • Nobody rebuilds them at 3am. In practice the arrangement fails silently and you sleep flat.

A foot-lift base fails at none of those. The deck is rigid, the shape does not change, and it is still there in the morning. That is the entire argument for a base in this context and it is a genuinely good one.

The zero-gravity position, specifically

The saved position marketed as zero gravity puts the knees slightly above the heart with the head slightly raised. For leg elevation it is a useful starting point: the legs are up, the knees are bent rather than locked, and the upper body is not folded. What the presets actually are.

You do not need a preset to reach it. Any base with an independent foot motor can be set by hand, and a memory button is a convenience rather than a capability.

What to buy, if you decide to

If two of you share the bed and only one needs the position, a split king means nobody compromises.

One more thing worth knowing: if elevation is needed because of a diagnosed condition, that may be relevant to hospital bed coverage under Medicare — the coverage criteria include positioning that an ordinary bed cannot achieve. It is a narrow route, but it is a real one and it is worth understanding before you spend your own money.

Questions people actually ask

How high should legs be elevated for swelling?

The usual guidance is above the level of the heart. In a bed that means the feet raised meaningfully higher than the chest, which is more elevation than a couple of pillows produce and considerably more than most people assume.

Do adjustable beds help with edema?

A foot-lift base holds a raised position all night without the support sliding away, which is what pillows fail at. Whether elevation is right for a given cause of swelling is a clinical question, not a furniture one.

Can I just use pillows under my legs?

For an hour on the sofa, absolutely. Overnight, legs slide off a pillow stack within an hour or two and you wake up flat. That is the specific problem a base solves.

Is leg elevation safe for everyone?

No. Swelling has many causes, and some — heart failure and circulatory disease among them — need medical management rather than positioning. New, one-sided or painful swelling needs a doctor, not a bed.
